Referrals
Referrals should have all relevant information filled out including contact details for making appointments. Accompanying all referrals should be any supporting reports or other documents to assist in triaging. This will ensure that the service agreement will reflect the services required to meet the referral. A service agreement will then be sent with a consent form. It is essential that the consent form also be signed along with the service agreement. If not signed there could be delays in accepting referrals.
